What Is Dialectical Behaviour Therapy?

DBT is a structured therapy that builds on cognitive behavioural therapy (CBT) but is used particularly for people who struggle to manage emotions and have developed unhealthy coping methods.

In DBT, you’ll learn to recognise that you have intense emotions and work on better ways to handle them. This is the crux of DBT: understanding what you are experiencing and the negativity it brings, whilst learning how to change the behaviours that are harming you.

How Can DBT Support Addiction Recovery?

When people experience emotions, they usually engage in some behaviour. If you struggle to deal with emotions, you can use unhealthy methods to cope, which could lead to substance abuse.

There are many situations and emotions which lead people to alcohol or drugs as a way to cope, including:

  • Stress
  • Emotional distress
  • Conflict.
  • Feelings of rejection
  • Anger
  • Anxiety
  • Cravings
  • Impulsive urges

DBT can help you recognise the links between these emotional triggers and the unhealthy coping methods. Research has found that DBT is useful for those with substance abuse issues, especially in helping improve responses to impulsivity.

DBT in alcohol and drug rehabs involves learning four key skills.

Emotional Regulation

This involves getting better at identifying emotions, understanding what may trigger them and recognising how they could be influencing your behaviour. You then work on healthier ways to cope with emotions.

Distress Tolerance

Not everything has to be coped with or immediately confronted. DBT can help you become more comfortable with experiencing difficult feelings or situations without losing control and seeking relief through alcohol or drugs.

This is helpful during early recovery, improving your ability to manage cravings and deal with stress. It can help you with interpersonal conflict, as it can reduce your desire to be defensive or stop escalation.

Mindfulness

Mindfulness is about being in the present moment, helping you to notice experiences, thoughts, physical sensations and emotions in real time. With this grounding, you might be less likely to focus on the past or future or get mired in negative thoughts, instead focusing on the smell of something or the sound of your breath.

This may help you overcome cravings and urges, directing your attention only to the current moment you are in.

Interpersonal Effectiveness

This is about improving your communication, getting better at asking for what you need and building up boundaries. This can help you improve relationships, manage conflict and ask for support when you need it.

Ocean was the first step in my journey to recovery. I arrived not knowing I was going in to rehab and absolutely terrified whilst experiencing horrific withdrawals and not knowing where I was. I can’t say enough about how compassionate and caring the team working at Ocean are. I am beyond grateful for the month detox in Ocean and the help the team gave me to find my next step in my recovery journey upon leaving. It really is a safe haven during the vulnerable detox stage and there were plenty of laughs to be had in my four weeks at Ocean. A huge thank you and I will be eternally grateful for all of the support - which continues in aftercare after leaving!

This place helped me grow and become more aware of my short comings yet still move forward. I found my tribe here. Every single member of staff had an impact on me from support team, amazing house keeping and Lee's morning disco in the dining room. I will treasure my time and the people who I now consider as friends. Thank you

I turned up as possibly the most reluctant guest ever to grace the doors at Blackpool. The patience of those who welcomed me was apparently second to none (although I have no memory). My experience was second to none, staff , brilliant, patient, understanding, wise. I can’t thank them enough for pretty much saving my life. If the staff read this they will know exactly what I have to be grateful for! Thanks you you all.

Spent 4 weeks at Ocean and it was like a holiday. Food is restaurant standard and its super modern and clean. It cost around 12 grand for 28 days in June 2022. I was an opiate painkiller addict and alcoholic and withdrawal from morphine a few years earlier was like going through a Heroine rattle and so i was very scared of suffering withdrawal. I suffered no pain or discomfort because of the cocktail of medication they gave me to detox. (Librium amongst others) The staff, are professional and know exactly what addicts go through. Every staff member I knew were recovered addicts. Walks on beach every day. Be aware, Caffeine is banned so becareful what you take in. I will admit 28 days without Caffeine was exhausting and painful so I snuck Jars of coffee in every week from the shop. I have bipolar so self medicating with Caffeine relieves the fatigue from low mood. We had a group between 7 to 15 within the month. About 6 people told the rest of us what other high charging centres were like and everyone said Ocean is the best. Your own room, private bathrooms and no indoor cameras. Most other centres watch your every move via a camera surveillance system. Ocean recovery allows mobile phones too. Many don't. Take it from me, this is the best option. Yes its costly. But you get what you pay for. I've been in recovery 9 months and my whole life has changed. Anyway, that's my experience

Who May Benefit from DBT for Addiction?

DBT may be used for alcohol addiction or drug abuse because intense emotional and behavioural difficulties can lead to substance issues.

It can be beneficial for certain people, including those experiencing:

  • Difficulty managing intense emotions – Those who get overwhelmed by emotions like anger can struggle to deal with them.
  • Impulsive behaviours – Acting without thinking about consequences can lead to repeated instances of drinking or substance use, without thinking about what this does in the long term.
  • Strong responses to stress – If you struggle to cope with stress, you may use substances as a way to do that.
  • Repeated substance use following emotional distress – Going back to substances whenever you feel distressed that recurs if the underlying issue isn’t dealt with.
  • Relationship conflict – Arguments, breakups and poor communication contribute to emotional distress.
  • Difficulty tolerating uncomfortable feelings – Those who struggle with feelings and want to avoid them seek immediate relief. Initially, substances can provide that.
  • Difficulty managing cravings or triggers without acting on them – If you struggle to resist urges, continued substance use can be more likely.

DBT and Relapse Prevention

Beginning DBT in rehab can be beneficial for the rest of your recovery journey. It supports relapse prevention because it can help you with recognising emotional triggers, getting better at regulating difficult feelings and developing healthier coping responses when you experience these emotions.

You’ll learn skills to manage distress, tolerate uncomfortable emotions and respond without going back to substance abuse.

This will help in the long term. It should help you improve your awareness in high-risk situations, such as parties or around others using drugs or alcohol, and inform plans you have made as part of your aftercare to cope during these moments.

You should continue to hone the skills you learn during DBT sessions in treatment on the outside, whilst engaging with wider aftercare such as weekly support meetings. When you leave Ocean Recovery, you gain access to a weekly aftercare group for up to 12 months after treatment.
